1. Select a general dentist from this directory. For ease of selection, we have listed general dentists geographically. 2. Contact your selected network dentist for an appointment and inform the dental office that you are a TDA-PPO covered member. 3. Present your Group I.D. card issued by your insurance plan. Although the I.D. card will not guarantee coverage it will serve to identify you as a member and provide information for submitting claims, obtaining eligibility verification and the dental benefits available through your plan. 4. If the services of a specialist are necessary, your general dentist can help you with the selection from the list of TDA-PPO specialists. 5. Always follow the procedures for filing a claim as outlined in your employee plan booklet/certificate. You will continue to be responsible for any patient coinsurance and/or deductible amounts to the dentist. For additional TDA-PPO dental network information, please call TDA at (801) 268-9740 or toll free 1-800-880-3536 When you or a covered family member choose to receive dental care from one of the participating TDA-PPO dentists, you will significantly reduce your out-of-pocket costs. Generally, your savings will be realized through enhanced plan benefits and our pre-established fee schedule with the dentists. |
